Ethical Dilemma Essay

Patient confidentiality has become an integral aspect of healthcare ethical standards since the HIPPA law came into being. (Erikson 2005). According to the American Nurses Association (ANA) code of ethics â€Å"the nurse has a duty to maintain confidentiality of all patient information† (Nursingworld, 2005). When a patient’s confidentiality is violated, his/ her wellbeing is negatively impacted. Patient confidentiality encompasses protecting any information the patient reveals to medical staff, and not divulging or sharing it to others. If trust is betrayed, the patient would be uncomfortable and reluctant to share their private and personal information. The nursing profession is built on trust and providing good care, and there are limited reasons to breach a patient’s trust. A breach of confidentiality means disclosing a patient’s information to a third party without the patient’s consent or an order from court and this may be done by phone, verba lly or electronically. If this type of disclosure of the patient’s information is shared to unauthorized persons, a breach of confidentiality results and this has ethical and legal implications. The law protects patient’s right to confidentiality and nurses have a moral duty to do the right thing for the patient. Understanding the ethical principles is very important because it provides the basis for nursing practice. According to the American Nurses Association, the ethical principles that underlie confidentiality are autonomy ( agreement to respect another’s right to self-determine a course of action; support of independent decision making), beneficence ( compassion; taking positive action to help others; desire to do good; core principle of our patient advocacy) and nonmaleficence (avoidance of harm or hurt ).These principles ensure that nurses act in a way that benefits the patient, causing no harm, respecting the patient’s personal information and promoting patient’s right to self-governing. It must however be noted that there are many situations that can arise in which patient confidentiality becomes a dilemma for nurses. Dilemmas around   confidentiality arise when the principle of confidentiality is in possible conflict with other ethical principles such as avoiding harm to the patient or others. The scenario below underscores this point: A day after he was admitted to the hospital, Fiifi was informed that he is HIV positive. The medical team advised him to contact his sexual partner and inform her of his status. For the past 15 months, Fiifi has been in a relationship with Derby and they are expecting a baby in 6 weeks’ time. Before this relationship Fiifi admits having a series of sexual partners. 5 days later, it became clear that he has not told Derby of his HIV status. Being aware of the baby, soon to be born, the medical team tells Fiifi that steps should be taken to assess whether Derby is HIV positive and whether the baby is at risk or not so that necessary treatment could be initiated. At this point, Fiifi was reluctant to tell Derby and even threatens to sue the hospital if she is told without his consent. What should the team do? This is a typical case of ethical dilemma; should the team inform Derby or what? A critical analysis reveals that the principle of autonomy requires that personal information should not be disclosed without consent. However, in some cases the autonomy of another person may also be at play as it is in this case Derby, the previous sexual partners and the baby when born. Not disclosing information may limit their ability to make decisions regarding treatment and lifestyle and thereby violating the principle of non maleficence. Even though maintaining confidentiality is crucial, the consequences of disclosing or not disclosing are also important considerations. In this scenario for example, the consequences of non-disclosure can be identified as, the risk that Derby may be HIV positive and the effect of not providing information to enable her to be tested is that she is harmed by not knowing her HIV status and not receiving a course of treatment. If Derby is HIV positive and is not aware of the risk, the consequences are that she will not take steps to minimize the risk of infection to the baby; For example, obtaining treatment during pregnancy or prophylactic treatment and knowing not to breastfee d. If Derby finds out later that there was a risk to her and that she was not informed, she may lose trust in the healthcare system. The scenario here presents a conflict in which the nurse is torn between an obligation to the patient and an obligation to the others (Derby, the baby to be born and the other sexual partners). Using the ANA code of ethics as a  frame of reference, The Code of Ethics for Nurses with Interpretive Statements (ANA, 2001), emphasizes the responsibility of the nurse to promote the welfare, health and safety of the public. Further, the code states that the primary commitment of the nurse is to the patient. This is like a double-edged sword. It must however be noted that there is an exception to the principle of confidentiality, which justified on the basis that a breach of confidentiality would help prevent harm to an identifiable person, and this is buttressed by the ANA’s provision that there are some exceptions to this duty such as a greater need to protect the patient and other parties (Nursingworld, 2012). Censorship and dehumanization in V for Vendetta by James McTeigue and in Brave New World by Aldous Huxley Government control over its citizens is significant to the regime and it is this dominance over their people that leaves little room to question authority, but rather conform to the regime’s ideologies. This is portrayed in V for Vendetta by James McTeigue and in Brave New World by Aldous Huxley. Both film and novel show significant aspects as to how the two governments use censorship by conditioning the citizens and being cruel where the society uses dehumanization as a means of control. The government is determined to create a stable and ideal society, however, the distinction between citizens is censored by the government, resulting in totalitarian rule. This portrays the negative influence a controlling government has on people. In order to prevent it, citizens must rebel against the authority and unite. The government censors community perspective and affects their social status. In both works, the controlling governments have a negative impact on the people by reinforcing laws and restrictions on the society, making it an ideal one. This is seen in Brave New World when the Director states, â€Å"the secret of happiness and virtue-liking what you’ve got to do. All conditioning aims at that: making people like their inescapable social destiny†. The Director is emphasizing how the World State has the higher authority over everyone in society. This is exemplifying the members of the community’s castes are â€Å"conditioned† at a young age and are instructed to perform daily tasks to create an ideal society. Regardless of what a human being is capable of, the World State aims to make â€Å"people like their inescapable social destiny† meaning their rights and freedom in society are reduced, however, they are conditioned to feel happy. In contrast, in V for Vendetta, the government is also censoring the community’s perspective and affecting their social status. This is evident in the film when V mentions â€Å"cruelty and injustice, intolerance and oppression. And where once you had the freedom to object, to think and speak as you saw fit, you now have systems of surveillance coercing your conformity†. Government has ultimately shown the use of totalitarian rule as of one aspect to govern the citizens. The use of â€Å"cruelty and injustice† from the controlling government is portrayed, meaning unfairness and savagery are being shown towards the citizens which â€Å"oppresses† them to express their feelings in society. These two controlling governments are both different because of the way censorship is used against the society. The government in Brave New World uses a method of â€Å"conditioning† to make people â€Å"like their inescapable destiny,† meaning citizens perform the desired task instructed by the government. However, in V for Vendetta, the use of â€Å"cruelty and injustice† is used in order for the citizens to perform their tasks as society instructed by the government. Ultimately, the way in which each government maintains control over their citizens differs, however the objective is the same. Through the oppression of their citizens they can gain a greater rule. Another way the government has a negative impact by the use of laws and restrictions towards the people in Brave New World when the Controller states â€Å"One believes things because one has been conditioned to believe them†, drawing attention to conditioning yet again. Citizens do not have the freedom of thought to question what it is they must believe, they simply are taught to blindly believe. Similarly, in V for Vendetta, the negative governmental impact is evident when V states, â€Å"a handful of oppressors have accepted the responsibility over our lives that we should have accepted for ourselves. By doing nothing, we gave it away†. It becomes clear that V understands that the freedom of thought would have given him power to fight against the government. In both works, the contexts are similar because in Brave New World, citizens are â€Å"conditioned to believe things† at a young age. Similarly in V for Vendetta, the government is reinforced to accept the ir lives in society which are controlled by a â€Å"handful of oppressors.† Both works show the negative governmental impact on society and affecting the social status of individuals conditioning and censorship which results in a totalitarian government in order to create the ideal society. This demonstrates how the government is determined to create a utopian society by using censorship and conditioning. However, the lack of distinction amongst the individuals is censored by the government. The government’s use of dehumanization and stripping of identity is vital to create a utopian society through methods of control. This is evident in Brave New World as there is a distinct caste system that dictates each member’s identity. This is achieved through a Bokanovsky process where there are Ninety-six identical twins working ninety-six identical machines†. This exemplifies how there is no such thing as individuality as the identity is assigned to them based on a caste and they are exactly identical. Through this process, the government successfully strips a citizen’s identity and is thereby able to control them more effectively. On the other hand, V for Vendetta gave a significant aspect as to how negative governmental impact can harshly strip a citizen’s identity. This was evident in V for Vendetta when Evey reads her Valerie’s letter in the cell she was captured in and referred to the cruel injustice Valerie suffered from the govern ment. The letter read, â€Å"They shaved off my hair and held my head down a toilet bowl and told jokes about lesbians†. This is explaining how Valerie was a lesbian and had feelings for her girlfriend named Ruth. However, Ruth killed herself because she couldn’t live with betraying Valerie. Later on, they came for Valerie who was also stripped down by the government officials and harshly abused her by â€Å"shaving her hair† and holding Valerie’s head â€Å"down a toilet bowl† while making â€Å"jokes about lesbians.† These events stripped Valerie’s identity because she loved an identical gender which was oppressed by the government and thus the government thereby is able to control the society and oppressing an individual’s feelings. These two ideas contradict because of the citizens in Brave New World are assigned an identity based on a caste however, in V for Vendetta, citizens are stripped for loving their identical gender by the government. The governments are urged to create a stable society with the Bokanovsky process and stripping identity of an individual in order to maintain control over its’ society Another example of dehumanization is seen when Bernard’s characterization is shown in the book. This was evident in Brave New World when the Director states, â€Å"if ever I hear again of any lapse from a proper standard of infantile decorum, I shall ask for your transference to a Sub-Centre-preferably to Iceland†. This is demonstrating the director is stating how the characteristics of Bernard, not being â€Å"infantile† are inhuman from director’s perspective, and thus he threatens Bernard to transfer him to Iceland due to his behavior. This is also shown in V for Vendetta when Evey is tortured by V, in the same manner that V was tortured by the government and how everyone is tortured by the government. This was evident when Evey was informed by the jailkeeper, â€Å"The penalty in which is death†¦you must tell the identity of V and you can live back to your normal life. If you cooperate, you will be released from this faculty†. Evey decides not to cooperate with the government and as means of control, Evey was forced to live in severe conditions by being tortured in the cell. This scene portrays the significance of government control by stripping identity of an individual if a citizen fails to â€Å"cooperate with the government.† The mention of â€Å"penalty in which is death† was to infiltrate Evey’s mind with fear. In other words, if an individual decides not to collaborate with the authority, there are consequences for every citizen. These two medias are parallelly similar in a sense that restrictions and laws are all applied to every citizen, however the consequences are quite diverse as in Brave New World, citizens are transferred to Iceland due to any inhuman characteristics whereas in V for Vendetta, the citizens are harshly abused if an individual fails to cooperate with the authority. The government’s ideology is to create a utopian society, however, if an individual disobeys the r estrictions by the government, he/she may face consequences that results in totalitarian rule. In conclusion, Brave New World and V for Vendetta compare in the sense that they both make their audience aware of the negative governmental impact both medias shown with the use of censorship and dehumanization. The government is determined to create a utopian society despite the lack of variation between citizens and the negative governmental impact it effected on the society. The use of censorship and dehumanization dominated all individuals and putting restrictions on their uniqueness and human qualities one may possess at birth.

Personal Goals Essay Setting goals should not be just for school assignments but should be part of one’s everyday life. Developing goals gives people something to strive toward and allows an individual to keep going forward in life. Without goals, it is very easy to give up when things get rough. According to Bishop, Carter and Kravitis (2011), â€Å"Setting goals involves defining what an individual is aiming for in both long-term and short-term time frames. Long-term goals are broader objective an individual wants to achieve over a long period, perhaps a year or more. Short-term goals are smaller steps that move an individual toward a long-term goal, making it manageable and achievable, piece by piece (p. 98). Smart Goals is a tool that helps to focus and create effective goals. Smart Goals is an acronym that consists of five steps: Specific, Measurable, Attainable, Relevant, and Time frame (Carter, Bishop, Kravits, 2011). Examples of using these acronyms for writing this paper are Specific – by receiving a B grade in every nursing class, Measurable – by completing each class for a step closer to receiving a degree, Attainable – by commitment and many hours of study, Relevant – achieving a B grade is not unrealistic with commitment and determination; the goal is to think positive and know that I can achieve anything, and Time frame – every five weeks completing a class. The following are the specifics of my personal and professional short and long-term goals along with the challenges in achieving them. Short-Term Personal Goals One of my short-term personal goals is to control my overspending. Most of my overspending is a result of depression, hence the compulsive purchases. The way I deal with depression is to go out and buy things. It makes me feel good for the moment but when I get the bill I become depressed and stressed. I believe in dealing with this issue will allow me a sense of peace. Since, I started back to school, there is no better time to start on this goal because attending school and studying is a tremendous distraction that will take up a majority of my time. My boyfriend manages his money very well and pays off all his credit cards every month and has no debt. I decided that I will have him help manage all my credit cards and bills to achieve my goal. The next step will be to cut up my credit cards and keep one for an emergency. Another strategy I am going to use, the days go out with my friends I will make them aware of my goal and solicit their help to keep me on track. When I am about to buy something, I will have them remind me about my goal of getting out of credit card debt. My milestone for this goal is one week to get the above strategies in effect. Long-Term Personal Goals One of my long-term personal goals is to pay-off all my credit cards. The first thing I need to do is to create a picture of how much debt and recurring bills I have in a month. This involves adding up all my credit card debt to see how much I owe, adding the monthly minimum payments, and adding up all my monthly recurring expenses: i.e. car loan, car insurance (divided by six), gas Family Fitness fees, ATT bill, and groceries. The next step is to add up all my income in a month. From here I can subtract the minimum amount due on all my credit cards and recurring bills from my income with an outcome of my maximum monthly income. With this result, I will decide how much more I want to contribute to paying down my highest interest rate credit card first, putting some away in my savings account for unexpected emergencies, and some for a little pleasure. The next step is to decide a realistic milestone to achieve this goal. I am giving myself one and a half years, my expected graduation date, so I have a clean slate to start paying off my school loan. To keep myself on track to my target date, I am starting an expense log that will allow me to monitor exactly where my money is going. By doing this I should be able to adjust my numbers to meet my goal. Another way to achieve this goal is by staying in school believe it or not. If I am in school and studying a majority of my time, I will have less time to go out and spend money. By doing and implementing all the above, I will be on my way to being debt free by the time I graduate. Short-Term Professional Goal One of my short-term professional goals is to complete the HCS/301 class with at least a B grade. My milestone achievement date is set for two weeks. To reach this goal I am taking several courses of action. The first is to manage my time. Managing my time will be challenging to say the least. I will be juggling my time between work, school, keeping up my home and spending a little time with my longtime boyfriend and dog. Oh wait, I forgot to list time to sleep. To do this, I will write down how many hours of work and commuting time I do each week (average 56 hours), time for boyfriend, dog, and home management (17 hours), and. time for school (39 hours). With my learning challenges, 39 hours per week is enough time for school. Another course of action is to form a study/support group. This group will help in discussions on schoolwork, to reinforce the in class material, and to discover different ways of doing and thinking regarding school assignments. A third way to achieve this goal is to take advantage of all the resources available at University of Phoenix. This would include using the university library, disability services, technical services, and taking workshops just to name a few. By consuming all these resources, I am sure it will reduce the amount of time and stress it takes me to complete each assignment. Long-Term Professional Goals One of my long-term professional goals is to obtain a Bachelor of Science in Nursing (BSN) degree. Working as a Registered Nurse (RN), I have experienced many roadblocks in obtaining another job and have fought going back to school for the past year. I finally have come to the realization that to make myself more employable in today’s job market that a RN with a BSN degree has a distinct advantage over a candidate with only an AA degree. According to Williams (n.d., p. 1), a study published in the Journal of the American Medical Association, and reported by the American Association of Colleges of Nursing, â€Å"surgical patients treated by greater percentage of nurses holding Bachelor’s Degrees had a survival advantage† over those patients who did not have nurses with BSN. As mention previously, I plan to succeed academically by continuing with my study/support groups, staying organized, continuing using resources at University of Phoenix, and completing each course in a the curriculum with at least a B grade. Challenges Everyone will face challenges and roadblocks throughout their lifetime. What sets up apart is how deal with them. If one allows it, challenges can stop us from achieving our goals. The major challenge I am facing at this time starting University of Phoenix is my negative thinking. I do not want to go back to school but have to out of necessity to keep my job. My negative thinking is due to my learning disabilities some of which are a lack of skills in reading and writing that seem to be a requirement in obtaining a BSN degree. Adding all these challenges together greatly contributes to my lack of confidence. A good support group at school and home is easing my negativity. Conclusion In conclusion, this paper was about personal and professional short and long-term goals. With each one, I discussed strategies on low to achieve each goal and the expected milestone for each. Reaching each milestone is empowering, to know that we are one step closer to achieving the next goal (Morisano, 2008). You have to make sure you are aware of what you are using and how it operates in your system. In today’s society there are a million anti-virus software programs. Many of them have free versions and some just have trail versions you can test drive before buying the full version. McAfee and Norton have trail versions you can sample before getting the full version. With Panda, AVG, and other anti-virus programs they have a free version and full upgraded version you can use. When it comes to viruses and the types that get put onto your computer there are many of them. For example: the Trojan Horse, it is put into your computer by saying it is good for file converting or cleaning a system out, when it is really putting bad programs into your computer along with spyware and other potentially harmful programs. Just like when you go into your email and you see an email with a suspicious name, you click it and it automatically downloads something into your computer and you computers starts to malfunction. When it does this it is because the files that were put on your computer are basically eating up you memory. It is extremely important to keep your computers malware and anti-virus programs up to date to prevent any type of program getting into your system. When you back up a system you have different types of backups you can use. For instances you have your full back up which is where you can back up all your information onto a DVD-R or DVD-RW, so you can recover your system at a later time is it crashes or is lost. You should also look at your back up disks periodically to make sure they are in good form if needed to be used. You also have your deferential back up is where you can select certain things to back up and incremental back up is where your computer sets up a certain time after you have put so much into your system to back it up for later use if needed. Checking back up disks are extremely important to check as well to make sure that they are able to be used on the future. Also it is really important to burn your files to a disk not on your hard drive in case they are lost or damaged. Five passwords that are good to use in the way of remembering and using are, â€Å"Bab1eg1r!†, â€Å"Crazy8ight$†, â€Å"GoP!a1†, or â€Å"Playf00!† They can be used to help protect your passwords by replacing a letter by a number and capitalizing some of the other letters that you choose to use, along with using symbols of some sort. Blacks joined abolition only on sufferance. They also joined at their own risks. For a long time, Douglass, a man of pride and artfulness, denied this fact. For years there had been disagreements among many abolitionists. Everyone had their own beliefs towards abolition. There was especially great bitterness between Douglass and William Lloyd Garrison, dating from the early 1850's when Douglass had repudiated Garrisonian Disunionism. Garrisonians supported the idea of disunion. Disunion would have relieved the North of responsibility for the sin of slavery. It would have also ended the North's obligation to enforce the fugitive slave law, and encourage a greater exodus of fugitive slaves from the South. (161,162 Perry) Douglass did not support this idea because it would not result in the complete abolition of slavery. Blacks deserved just as much freedom as whites. He believed that the South had committed treason, and the Union must rebel by force if necessary. Astonished by Garrison's thoughts, Douglass realized that abolition was truly a war between whites. Garrison, and many others, had failed to see the slaves as human beings.   Ã‚  Ã‚  Ã‚  Ã‚   Were blacks then supposed to be irretrievably black in a white world ? Where is the freedom and hope if all great things are privilege only to the whites? Douglass resolved never again to risk himself to betrayal. Troubled, Douglass did not lose faith in his beliefs of abolishing slavery. However, he did reinvent his thinking.   Ã‚  Ã‚  Ã‚  Ã‚  Douglass eventually made his way with what amounted to the applied ideas of Alexis de Tocqueville and Fancis Grund, both of which were writing at the time when Douglass realized the truth about abolition.
